Service Agreement

S.S.P.M. agrees to provide Weekly Full Service Maintenance plus Filter Cleans, Weekly Full Service Maintenance, Weekly Chemical Only/1 Clean per Month Maintenance, Semiannual Filter Cleans, or Other Maintenance in accordance with the terms set forth below. The initial term of this contract is on a month to month basis, and can be cancelled by the Customer with a written notice effective thirty (30) days after the written notice date. S.S.P.M. reserves the right to cancel or refuse service at any time. Credits for absence of service due to inclimate weather or customer equipment failure (both to be determined by S.S.P.M.) will not be issued.

GOLD $230 *Weekly Full Service Maintenance with D.E. Filter Cleans & Insurance*
S.S.P.M. will provide all chemicals, test chemicals in water, backwash applicable filters, clean all baskets and debris traps, brush, net, andvacuum pool when necessary, leave notification of service, tify Customer of any pool/equipment issues, clean D.E. filter in April &October, and Customer receives 10% off all repairs. Customer is responsible for maintaining appropriate water level in pool.

Silver $190 *Weekly Full Service Maintenance*
S.S.P.M. will provide all chemicals, test chemicals in water, backwash applicable filters, clean all baskets and debris traps, brush, net, andvacuum pool when necessary, leave notification of service, and notify Customer of any pool/equipment issues. Customer is responsible formaintaining appropriate water level in pool.

Economy $129 *Weekly Chemical/1 Clean per Month Maintenance*
S.S.P.M. will provide all chemicals, test chemicals in water, backwash applicable filters, clean all baskets and debris traps, leave notificationof service, notify Customer of any pool/equipment issues, and provide services listed under Full Service Maintenance one (1) week out ofthe month, providing pool is not excessively dirty, (to be determined by S.S.P.M.). Customer is responsible for maintaining appropriatewater level in pool.

Semiannual Filter Cleaning:

Add Semiannual Filter Cleans to any Service Plan for $16.50 per month.
S.S.P.M. will perform two D.E. filter cleans per year (April & October). *Customer understands that by refusing the D.E. Filter Clean option they are running the risk of damaging their D.E. filter. All major D.E. filter Manufacturers recommend the filter be serviced semiannually. S.S.P.M. will not be responsible for D.E. filters that are damaged from lack of Manufacturer’s recommended maintenance. Initial ______*

Payment Options: *

Auto-Charge: (S.S.P.M. holds Customer’s major credit card number in their secure database. Customer is automatically billed on the 20th of each month. Any extra charges that exceed the monthly service rate must be approved by Customer before card is charged.)

Mail-In Option: (Customer mails in check or money order in supplied return envelope. A $5.00 Processing Fee will be added to each monthly statement when selecting this option.)

Customer will be billed $ plus applicable sales tax a month for selected services. All services are prepaid, and services will not be performed in the event of non-payment. The Customer will be billed on the 20th of each month, and will have ten (10) days to submit payment to S.S.P.M. Any balances not paid by the 5th of each month will incur a late fee of $15.00 applied on the following monthly statement. There is a $30.00 processing fee for all returned checks. If S.S.P.M. is unable to collect fees, the company will take legal action and file a claim against the Customer, and that Customer will be responsible for all fees, court costs, and liens. S.S.P.M. reserves the right to increase service rates and change the day of weekly service without notification to the Customer. Any separate services performed by S.S.P.M. (parts, repairs, equipment) will be invoiced separately and payable upon receipt. All payments will be mailed to P.O. Box 126560 Benbrook, TX 76126.
